Clinical Supports

What are Clinical Supports?

Our clinical supports help you look after your health, manage your personal care needs and maintain the abilities that support independent living. Whether you need nursing care, assistance with daily routines or help coordinating your services, we work with you to understand your needs and what matters to you. Your support is planned around your health, goals and preferences, with care delivered by appropriately qualified and trained workers. Where clinical supervision is needed, we arrange this to help ensure your care is safe and suited to your circumstances.

Nursing Care

Our nursing care brings clinical support to you at home, helping you manage your health with confidence. Depending on your assessed needs, this may include assessing and monitoring health conditions, administering medication, wound care, clinical continence care and maintaining skin health. Our nurses can also help you and the people supporting you understand your care, recognise changes in your condition and connect with specialist services when needed. Nursing activities are undertaken by workers with the appropriate qualifications, skills and supervision for the care being provided.

Includes:

  • Services provided by registered nurses

  • Services provided by enrolled nurses

  • Services provided by nursing assistants

Personal Care

Our personal care services provide respectful assistance with everyday routines such as showering, dressing, grooming, eating, toileting and moving around. We take the time to understand how you like things done, protect your privacy and support you to do as much as you can for yourself. Depending on your needs and Care Plan, we can also assist with self-administration of medication and non-clinical continence care, including changing continence aids and helping you access advice or funding. Medication support is provided within each worker’s training and authorised responsibilities.

Includes:

  • Assistance with selfcare and activities of daily living

  • Assistance with the self-administration of medication

  • Continence management (non-clinical)

Care Management

Your Care Manager works with you to plan, coordinate and review services that support your wellbeing and independence. Together, we develop your Care Plan, discuss your budget and preferences, arrange your services and check that your support continues to meet your needs. Care management also includes helping you understand your options, supporting you to express your wishes and connecting you with other services or health professionals. If your needs become more complex, we arrange appropriate clinical input or oversight.

Includes:

  • Home support care management
